Sports Physicals

REMINDER:

Any JH/HS student participating in an EMF activity are required to have a sports physical prior to the start of fall practice on Monday, August 12th.

*Your student will not be able to participate in practice until the physical and NSAA release forms have been received in the school office.*

Please reach out to the office with any questions.

Forms are available in the office during normal business hours or on the school website.
